Position Summary B2H provides community based services to children who are in the care and custody of a LDSS or OCFS and who have significant mental health care needs, developmental disabilities or medical fragility and who require an institutional level of care.
Initially, children must be in foster care and categorically eligible for Medicaid to be considered for enrollment in the B2H waivers.
Major Responsibilities
- Provide one-to-one Crisis Intervention, to de-escalate clients during crisis situations, provide crisis resolution support and develop a Crisis Stabilization Plan;
- Assist in coordination with the Health Care Integrator, with additional crisis response services to resolve the immediate crisis quickly;
- Make recommendations for revisions to the detailed plan that is developed by the Crisis Avoidance, Management and Training Specialist;
- Engage/link the enrolled client to other services and resources;
Utilize Agency phone and email systems following standard customer services protocol. This includes (but is not limited to): answering the telephone in a timely and professional manner, directing calls appropriately and returning phone and email requests promptly. Demonstrate excellent phone etiquette and customer service skills. Also demonstrate knowledge of agency programs and services;
- Maintain positive working relationships with program staff and others within the agency. Demonstrate strong interpersonal skills during interactions with internal and external customers;
- Possess a working knowledge of the Care Manager software.
- Model and practice sensitivity, fair treatment and acceptance of diversity in all interpersonal interactions;
- Participate and contribute to program Quality Improvement process;
- Maintain and submit all of the required documents and comply with all aspects of the Vehicle Safety and Usage Policy; and
- All other duties as deemed appropriate.
Minimum Requirements:
- Masters degree in Education, or a Masters Degree in a related Human Service field plus one year of experience working with children with special needs and their families.
- Must have means of transportation to travel throughout Erie County
- Must have a valid New York State driver’s license
- Must be flexible to meet the needs of children and families in their homes
- Must be able to work independently with minimal supervision
- Must process adequate computer and oral communication skills
