Intensive Case Management (ICM) Wraparound Program
The ICM Wraparound Program serves children and youth who are under the age of 18
and diagnosed with an emotional, behavioral or mental disorder. The goal of ICM
Wrap is to provide families with a community-based alternative to counseling for
their children who may be at-risk of out-of-home placement.
Through an individualized care model, ICM Wrap works to engage the child, family
and other care service providers to address the unique needs and strengths of each
family. While similar to the Child & Family Services Home and Community Based Services
Waiver Program, the ICM Wrap reaches out to a broader population by serving children
as they transition from adolescent to adult services. This process develops a care
plan, customized to fit the needs of each family, implements the plan, and evaluates
the process throughout a period of time working collaboratively with all vendor
services involved. Some of these vendor services that are offered through ICM Wrap
include:
- Care Coordination
- Respite;
- Family support;
- Crisis response services;
- Skill building;
- Intensive in-home services;
- Tutoring;
- Pre-vocational services;
- Mentorship;
- Therapeutic recreation;
- Expanded respite services including out-of-home respite; and
- Challenged-based camp and outward bound experiences.
